Streptococcal Infection and Rheumatic Fever, the Importance of Medico-Social Factors in Child Population in Bosnia and Herzegovina

In the three-year prospective study on the territory of Bosnia and Hercegovina we examined the conditions of arising and incidence of streptococcal infection of the upper respiratory tract, as well as the prevalence and incidence of the some other unpurulent complications of streptococcal ethology with the children of the pre-school age and the children of the school age.
The children examined come from five localities (Sarajevo, Tuzla, Derventa, Mostar and Banja Luka). The locality of Banja Luka is not included here due to the difficulties in our work caused by the earthquake in 1970. The obtained results show high incidence of streptococcal infections, high ratio of "healthy carriers" of Beta haemolic streptococcus among the children examined also a great number of patients with rheumatic fever and rheumatic heart disease, also with Sytonsillocardiale in the localities: Derventa and Sarajevo.
The reason for such a high incidence of streptococcal infections as well poststreptococcal unpurulent complications must be looked for in overcrowded classrooms, the school work in three shifts (Derventa) as a minimal housing room. They are certainly the reason why the high index of streptococcal infections group "A" is permanent with age school children who probably carry the infection to their younger brothers and sisters. No doubt that the other socio-epidemiological conditions of the environment contribute to this.
Though the above results are not definite they point out that problem of streptococcal infections and as well as healthy carriers and poststreptococcal complications in the examined areas is very serious, and deserves a permanent attention of paediatric health Service.
In this study Several groups of medical workers were included and they were acquainted with the problem and enable the organisation of unique Service fighting against Rheumatic Fever and rheumatic heart diseases in Bosnia and Herzegovina.
